It seems to be broken, so replacing it with a new copy of the default seems like the most helpful path.

Make sure your devices are in sync.

Change the name of your current note type to add “broken” to it –
Manage Note Types > Rename > "broken Image Occlusion". -
Make a new note type –
Manage Note Types > Add > "Add: Image Occlusion"– and leave it named that. -
In the Browse window, search for
note:"broken Image Occlusion"to find anything using that broken note type – select those cards/notes –Notes > Change Note Type– change them to use the newly created type, and make sure the fields (and cards) are mapped correctly. -
Get rid of the “broken” note type so you don’t use it again –
Manage Note Types > Delete. -
When you sync, you’ll need to upload to AnkiWeb and on your other devices, download.
