After creating an image occlusion enhanced card (after watching the tutorial: How to use Image Occlusion Enhanced add-on for Anki (Memorize any Picture) ), I only get an icon as a question card.
It looks like you deleted your “Image Occlusion” (or whatever that would be localized) note type. You can re-add that by clicking Manage [or Manage Note Types] > Add > Add: Image Occlusion.
I understand that none of this “solves” the issue you were having with that IO Enhanced note, but if you create your notes with the built-in functionality, you won’t need to.
First, delve into standard occlusion to gain a good understanding of the strengths and weaknesses of custom add-ons. In any case, a standard solution should be supported for years, while an add-on can be abandoned.

But you should try it after you’ve mastered the standard solution, and then evaluate whether you really need it or need something else.
there should be some recommendation against using this addon nowadays. it was relevant back in the days when there wasn’t a built-in image occlusion, but i see people who keep using it not knowing there’s a built-in option just because it’s in some old tutorials and running into issues very similar to our friend here, because this addon is now 4 years out of date.
We don’t have any way to control what other guides/tutorials/users recommend.
The add-on is still relevant and necessary for anyone who already has IO Enhanced notes that need editing (even if they are newly importing those from a shared deck). There are other users who continue using it for new notes, because that’s what they are used to, or because it includes a feature they like. [I don’t think that this particular add-on being 4 years old necessarily means there is anything wrong with it.]
There’s no reason for a new user to start out creating new notes with it, and especially if it means they need to troubleshoot issues. But that is naturally discouraged for anyone who spends time getting to know the app before they go looking for things to add to it – e.g. Getting Started - Anki Manual .
