Anki showing empty cards

Hi. Anki is showing me empty cards in the reviewer window – they come up as blank with a field to type in. It’s happening with the Picture Word type of note where the spelling field is blank. When I run empty cards, it reports no empty cards to delete. This is happening on both desktop and AnkiDroid. Try moving {{type:Always Back}} before {{/Root Form (Test Comprehension if here)}}
