Again and Hard buttons not working properly

When I press the again and hard buttons, the action on the card is as if I pressed the good card button.

I have disabled all add-ons, downloaded the most up-to-date version of both Anki and MacOS, closed and re-opened the app, re-booted the laptop several times, reverted to the previous Anki release. After all these fixes, the cards seem to work properly for approximately 5-10 cards before reverting to the improper format.

Currently running on MacOS Tahoe 26.3 and Anki 25.09.

Please help!

1. Are you using the mouse to click the buttons, the keyboard to press shortcuts (and if so, which ones), or an external input device (like a remote)?

2. What are you seeing that makes it seem like that?

3. Have you checked the Card Info to see the review history of the card after you grade it? You can select More > Previous Card Info without leaving the study screen.
