Hi at all.

I study medicine in Italy: could you suggest me the best setting for university exams?

I don’t know if the standard settings go well with exams (I study medicine).

There are no „best settings“. Try the standard settings and see how they fit your needs. Studying - Anki Manual and Deck Options - Anki Manual should be studied, there are also helpful videos when it comes to tweaking your settings: E.g.
