Looks like the error is caused by the add-on Image Occlusion Enhanced. Might as well be any other add-on, since the Anki version you’re running is so old, it probably isn’t supported by any major add-on anymore.

You could try installing the latest Anki version (2.1.61) from Anki - powerful, intelligent flashcards.
