I just optimized my parameters and resceduled cards and my reviews went from 146 to 268 is this normal?
I am on latest version FSRS -6 of Anki
I know i should optimize after my reviews double or every month
but this time i optimized
2 3 times in this week maybe is this a problem?

That’s fine too. Just don’t reschedule too often, it’s not worth it.
So the algorithm determined i need to see all these cards?
I do add 150 new cards into my review cycle since i am in medicine

Yeah, it determined the retrievability for the cards.
IIRC yes, the issue was with some version from the start of this year. You should be using descending R though, that’s better when in backlog.
Or even better IMO, use easy cards first. This gets you the best seconds per memorised card.
Yeah the reason i have it on ascending retrievibility since my mind is fresh in the start of my study session.
Backlog isnt a problem i complete my cards daily.
