Despite all of this, the cards still won’t give me true randomization. Has anyone encountered this before, and is there a deeper fix or setting I might be missing to ensure each time its random?
I see no issues. But you should keep the “insertion order” to sequential.
Can you give some more info about this? Which cards are you not seeing randomised — learn, review, or new? See Deck Options - Anki Manual for what those terms mean.
You will always see learn (red) cards before all the other cards if that’s the cause of confusion. And if something with those “red” cards changed, it was probably due to learn ahead limit. If it’s not this, please elaborate further on your issue.
What does “true randomization” mean to you? The problem with random, is that it’s random and you might be seeing patterns where there are none.
But if you can tell us more specifically what seems not random (un-random?) about the order of the cards, that might help.
And since you gave us a screenshot from Browse – are you asking about the order the New cards are listed in the Browse window, or the order they are presented in your study session?
You can leave that set however you want. It only impacts what happens when you add notes, not when you are studying existing cards/notes.
When I say not random I mean when I click on the deck and select ‘study now,’ the first card is always x, the second card is always y, the third z and so on.

But after you study and grade cards x, y, and z, they shouldn’t be available to be introduced as New again. Are you studying them in a Filtered deck that isn’t scheduling them? Otherwise, you shouldn’t see them again until their step delay is over.
In the past, AnkiDroid used to randomize the card order each time I tap on a deck. So if I tap on deck X in the deck picker screen, I get card A, then if I go back and tap on X again I get another card B (unless there is no other card, or it just randomly picks A again).
Basically, the order of cards was randomized each time I tap on a deck.
After some update, this behavior changed, and now it seems to randomize it once and “remember” the random order. It was weird at first, but I think it’s much better since I accidentally go back sometimes and I’d have to do the review one more time later.
If this is what you mean, then I can offer you two alternatives:
Make a filtered deck which you can rebuild to randomize the order.
Change the sorting order in the deck options, save it, then change it back to random.
If that’s the issue – I believe what you’re noticing is the “stable” study queue that came with the v3 scheduler. It’s built for the cards that will be shown to you today, and it doesn’t constantly rebuild like v2 did.
