Anki keeps on crashing as soon as I add cards

My Anki keeps on crashing as soon as I click on “add” for a new card. Already tried restarting it while pressing shift, this time I managed to add 3 cards before crashing yet again.

This is the error code:
A fatal error occurred, and Anki must close. Please report this message on the forums.
